New Patient Registration
If you live in our practice area and would like to register with us, please call in to the practice and enquire at reception where you will be given the necessary registration forms and pre registration health questionnaire to complete.
Please note that you do need to complete both forms and provide your NHS Number to register with us. We also require some form of photographic ID and the person wishing to register must present the ID in person
You will be registering with the practice rather than a specific GP.
Medical treatment may be available from the date of registration, however an appointment to see a Clinician is only possible after the registration has been processed. Please speak to a member of the reception team for further information.
Patients requesting to join the Practice list will not be discriminated against on grounds of:
1. Race, gender, social class, age, religion, sexual orientation or appearance.
2. Disability or medical condition.
New patients will be asked to attend a New Patient Check consultation with the practice nurse. Please bring your list of medications with you to this appointment, preferably a print out from your previous Drs.
Sometimes it may take a while ( roughly 6 weeks but can be longer if moving from outside of Wales ), for your full medical records to arrive, therefore you may be asked some questions about your past and present health, and any treatment you are taking.
We encourage all our new patients to kindly register with the NHS APP for online access to the Practice. You can use this service to book nurse appointments, order repeat prescriptions etc.
